Your child will only be released to individuals listed on this form. Please list anyone, in addition to the parent/guardian above, who is authorized to pick up your child at the end of each camp day.

Please provide an emergency contact who is different from the parent/guardian listed above. Walk Intuit staff will attempt to reach the parent/guardian first; this person will be contacted if we are unable to reach you.

Consents & Agreements

Medical Treatment Consent: In the event of an emergency, I give Walk Intuit Inc. and/or its affiliates permission to seek necessary medical treatment for my child if I cannot be reached in time. Walk Intuit staff will make every effort to contact the parent/guardian and emergency contact before any medical decisions are made.

Photo & Media Release: I give Walk Intuit Inc. permission to use photographs and/or video taken of my child during camp for promotional purposes, including the Walk Intuit website, social media, and printed materials. Photos will not be shared with your child's full name or other identifying information without additional consent.

Camp T-Shirt
Every camper receives one complimentary Walk Intuit camp t-shirt to wear on-site during their session. Wearing the camp shirt each day is required while on the property — it helps our staff and volunteers quickly identify campers for safety purposes.

Walk Intuit Inc. Kids Horsemanship Camp is a community educational program, not a clinical or therapeutic service. Participation in this camp does not constitute therapy and is not a substitute for mental health treatment. Camp programs are facilitated by associate marriage and family therapists, trainees, and experienced horsemanship professionals, all working within a trauma-informed framework to ensure children have a safe, supportive, and educationally enriching experience.
